Supportive Housing Services in Hartford and Middletown

Mercy Supportive Housing Services combines a rental subsidy and supportive services for chronically homeless persons with very low incomes, serious and persistent conditions including substance abuse, HIV/AIDS, mental illness or co-occurring disabilities. Case managers provide ongoing support and assist clients with daily living tasks as well as goal setting and life skills.  The goal of supportive housing is to help clients be successful in permanent housing, and give them a place to call home.

Emergency Short Term Housing

Emergency short-term housing is available for individuals with one or more specific co-occurring disabilities.

 

Last year, Supportive Housing:

  • Provided case management support for 141 households in scattered-site apartments in the Hartford and Middletown Counties
  • Assisted 100% of residents to participate in treatment
  • Worked with residents to increase their life skills through workshops and one on one meetings with their case manager
